Portfolio: My Profile

In this portion of your portfolio, you will gather important information and documents to stay organized. This section of your portfolio can also be used as a reference for you when talking with school and college personnel and when choosing and applying for college.

Document checklist

Ask your teachers, guidance counselors or parents to help you complete the My Profile worksheet and gather the following documents. After you have the documents below, place them in your portfolio behind the “My Profile” tab.

• My Profile worksheet
• Psychological and educational evaluations (if appropriate)
• IEP Transition plan
• Medical records (if appropriate)
• Summary of performance
• Age appropriate transition assessments (e.g., work samples)
• Resume
• High school transcript
• Letters of recommendation
• SAT or ACT scores
